Residential Services

Clearbrook offers residential services in 57 locations. Services range from smaller homes for up to eight people to an 80+ resident Intermediate Care Facility with round-the-clock nursing care. Services and supports are matched to the residents based on their choice and individual needs.

  • Each resident has an Individual Service Plan (ISP) that recognizes their needs and preferences and guides the support they receive.
  • Residents have access to a wide variety of clinical supports such as nursing, dietary, behavioral, psychiatric, physical therapy, occupational therapy, speech therapy, and licensed social work/counseling.
  • A full-time activity manager is on staff to assist in utilizing a full range of indoor and community resources.
  • The focus is on self-advocacy.
  • Social and peer development is promoted.
  • Residents get to decorate their own homes and bedrooms - making their house a home.

Community Integrated Living arrangement (cila)

For individuals who live in a Clearbrook CILA, we strive to offer opportunities for them to be as independent as possible in their home and community. With 57 CILAs, throughout 16 different communities, each residential home is integrated with the neighborhood and community.

Intermediate Care Facility (ICF)

Clearbrook’s ICFs have 16 – 85 residents who have their own bedroom or share a bedroom with one other person. Goals are developed for each resident and support is received in the areas of self-care, academics, community living, self-medication, and workshop skills.

Lattof Commons

The Lattof Commons in Rolling Meadows is our largest ICF.  It is home to 85+ residents.  There are rooms for both men and women, shared and private.  Personalized quality care is provided by trained staff with 24/7 access to nursing support.

Wilke

The Wilke home is located in Rolling Meadows and is home to 16 female residents.  This home has on-site nursing staff for a minimum of 20 hours per week.  During the day, residents attend one of several day programs provided by Clearbrook.

Fairfax

The Fairfax home is located in Rolling Meadows and is home to 16 male residents.  This home has on-site nursing staff for a minimum of 20 hours per week.  During the day, residents attend one of several day programs provided by Clearbrook.

Wright

The Wright home is located in Gurnee and is home to 16 male and female residents.  This home has on-site nursing staff for a minimum of 20 hours per week.  During the day, residents attend one of several day programs provided by Clearbrook.

The program's mission is to create opportunities for our individuals to be as independent as possible in their homes and their communities.

Director – Directly responsible for the operations and administration of all facilities and their staff.

Coordinator – Directly supervises the QIDPs.

Qualifies Intellectual Disabilities Professional (QIDP) – Responsible on a day-to-day basis for ensuring the integration, coordination, and monitoring of the active treatment program for residents. The QIDP directly supervises the Residential Counselors.

Residential Counselor (RC) – Provides direct support and services to residents and ensures their well-being.

Program Scheduler – Responsible for management of scheduling staff shifts, menu development, grocery ordering, and monitoring and registering staff for training.

Activity Manager – Develops, coordinates, and facilitates resident activities.

Registered Nurse – Ensures that medical and dental needs are monitored and met. Provides medication training to staff per DHS guidelines.

Medical Driver – Transports and assists individuals with medical appointments.
