Intermediate Care Facility for Individuals with Intellectual Disabilities (ICF/IID)
For some clients, an Intermediate Care Facility for Individuals with Intellectual Disabilities, commonly known as an ICF/IID, is the best residential option for them. Clearbrook’s ICF/IIDs have 16 – 85 residents and most either have their own bedroom or share a bedroom with one other person. Goals are developed for each client and programming is received in the areas of self care, academics, community living, and self medication and workshop skills. There is a nurse for the program on staff, at least 20 hours per week and up to full time, who coordinates all of the clients’ medical needs. During the day, the residents attend one of several of Clearbrook’s day programs. As much as possible, community integration is encouraged and often clients go on outings at different events in the area, especially on the weekends.

- What is the staff to client ratio at your program? One direct care staff cares for four clients during evenings and also during waking hours on weekends. Two staff members are scheduled in the home at night.
- What is the age range in the homes? The ages of the clients that reside in Clearbrook’s ICF/IIDs range from 21-80.
- Can an individual live in an ICF/IID and still attend school? Yes. The local school district will transport a client from an ICF/IID to local high school or special education program as long as the individual is at least 18 years of age.
- Can a client bring in their own furniture and decorate their room the way that they would like? Yes. As long as all the regulations are met, a client can bring in their own furniture and make their own decisions on decorating their room.
- Can I take my family member home for a visit? Yes. Clearbrook does encourage home visits however, it is also important for individuals to spend some time in the program in order for the individuals to make progress on their goals.
- Do I have to take my family member to doctor’s appointments myself? No. A staff member is assigned to take clients to local doctors’ appointments.
- What is the admission process? The family and/or guardian should have their local case management agency send a packet to Clearbrook’s Director of Admissions. Family members are then encouraged to tour the home. The case manager (QIDP) will arrange a pre-screening meeting to learn more about individual needs of the client. When a bed opens at an ICF/IID, and if the opening is an appropriate match for the client, a trial visit will be arranged.
Staff Positions and Primary Responsibilities
Direct Support Person – Provides direct care for the clients, oversees all of the programs for clients such self care and academic training.
Nurse – Coordinates the medical care for residents, assess the clients when they are sick and meets weekly with the doctor if the client has medical concerns.
Cook – Prepares most of the meals for the clients according to their diets.
Maintenance Staff – Ensures that the property is well maintained and also is responsible for making sure that maintenance emergencies are resolved quickly.
Medical Driver – Drives clients to their local medical appointments.
Qualified Intellectual Disabilities Professional (QIDP) – Develops most of the goals and advocates for the client. She/he is the liaison with the families.
House Manager – Supervises the direct care staff and arranges activities for the clients.
Residential Service Directors – Supervisor of the Qualified Mental Retardation Professionals and is in charge of the building when the administrator is absent.
Administrator – Manages of all of the staff and ensures that the program follows all the required regulations and procedures.
