Our Home Based Services (HBS) program is designed to help participants continue living with their family or in homes of their own, while providing the support needed to achieve their personal goals. Clearbrook works with families to help participants focus on goal-setting, develop daily living skills, learn behavior management, and develop other essential skills needed to achieve greater independence. By working together, we can make a meaningful difference in the lives of those we serve.

embracing New Opportunities
All Nikko’s mom wants is for him to be safe and happy.
The support Nikko received from the Transition Outreach Specialist gave his family confidence that Clearbrook was the right choice for their son.
Nikko graduated from high school and participates in a work-study program at Old Navy. Alongside these accomplishments, he has learned how to better manage challenging situations more effectively through breathing exercises. These techniques have helped him develop social skills for his future workforce transition, helping him approach new opportunities with confidence.
Today Nikko is thriving in a LIFE Transition Program and his future is filled with endless possibilities.


LIVING LIFE TOGETHER
Erin and Erich are a testament to the independence fostered by Clearbrook. With the support of Clearbrook’s Home-Based Services, they thrive as they live independently in the community. Their home is conveniently located within walking distance of the bus stop, making it easy to navigate town. They enjoy biking, bowling, and bocce ball, and Erin proudly competes in the Special Olympics, with medals to show for it.
Erin’s mom feels confident and secure about Erin's future, knowing that Clearbrook’s essential services are in place to support her.
