EXHIBIT HALL will host a variety of providers specializing in serving individuals with a disability & their families. Area’s of expertise represented will include adaptive equipment, behavioral & clinical services, benefits, durable medical equipment, employment, fiscal employer agents, home and vehicle modifications, housing, legal, family supports, post-secondary education, social services, training and much more.
ENTREPRENEUR HALLWAY will feature individuals with a disability who will be promoting their business. *Products will be available for sale during the exhibit hall hours.

Person-Centered Planning (11:00AM-12:15PM)

This session will provide an overview of person-centered planning including the CMS requirements for this process, as well as principles and practices that promote effective and meaningful plans for people receiving services.
Who, What, When, and Why of Person Centered Planning in Illinois (12:45PM-2:00PM)
Kit O’Brien Cota and Andrea Medley, Department of Human Services, Division of Developmental Disabilities
This session will cover Person Centered Planning in the Illinois Developmental Disability service system. Attendees will learn who this applies to, what does it mean, when did it take effect, and why has Illinois implemented these practices. The presenters will also include an overview of the Discovery process, Personal Plan, Implementation Strategies, lessons learned from the first year, and vision for the future.
