Life Book
Clearbrook is pleased to present you with a copy of the Life Book. Developed by a group of Clearbrook parents and staff, the Life Book is intended to be a resource that can assist families in discussing and planning for the future of a family member with special needs. Unlike other assessments or planning documents, the Life Book creates a picture of a person's life.
Families who have completed the Life Book have remarked that the process of responding to the questions has given them the opportunity to think and talk about their family member from a very personal perspective. It truly is about the life of the individual.
This Life Book is yours to complete and to share with whomever you wish to have the information. Families who have completed the book have suggested that it be done over a period of time so that it is not an overwhelming process.
